CAMP2012 CAMP FORMS (required for all accepted campers before June 1)

Note: Please mail all forms to the address in camp forms. You don't need to send all forms at the same time. To track your camp forms status please use the link: On-line Form Receiving Status Check . Please also note that Health Form B may be substituted with Physician’s Health Report Form dated within 1 year of camp session with full immunization record. Please write down your registration/application ID on ALL the forms so that they can be properly handled and please DO NOT staple forms together.

NECYSC is in compliance with 105 CMR 430.00 (State Sanitary Code, Chapter IV, Minimum Sanitation and Safety Standards for Recreation for Children)
Volunteers of NECYSC who have unsupervised contact with children are subject to a Criminal Offender Record Investigation (CORI)
and Sexual Offender Record Investigation (SORI) as required by M.G.L. c. 6, § 172G
NECYSC is granted a license and permit to operate a camp for children in the Town of Weston.
